Avera Credentialing Transition Update
Provider or credentialing contacts that would complete a January 2026 reappointment application for privileging at a facility client of Avera CVS and/or participation with Avera Health Plans. As shared previously, we are in the process of transitioning from Compliance Watchdog to symplr CVO as our credentialing vendor.
While the full transition date for software will occur in early October, it is necessary to send January 2026 reappointments now so that the files can be primary source verified and taken through the committee and board approvals before the end of January 2026.
Because we are transitioning to a new credentialing vendor,
- Reappointment applications will be sent via email rather than the electronic process you are accustomed to. You will receive this from sprovidercvo@symplr.com.
- You will complete the fillable MN Uniform Reappointment credentialing application and will need to print the pages that require signatures (including authorization and release for med staff applications).
- When the application and additional forms have been completed and signatures are complete, please scan and email the application and all documents to sprovidercvo@symplr.com.
